Workflow error "Workflow Error The data source control failed to execute the insert command"

I was building this designer workflow the other day when I got this error -> "Workflow Error The data source control failed to execute the insert command." while initiating a workflow.

I was testing the workflow when I got this error. I followed this post to resolve the issue and did resolve the issue, but it happened again. So I looked carefuly and realized that two versions of same workflow was running on the same item.

I really dont know how! So I deleted the item and removed the item and reattached the workflow to resolve the issue.
